From my understanding Utumno was a underground fortress? Or was it more a realm or another dimension?

We might have been able to call Utumno a realm in the geo-political sense, just as Angmar was a realm, but even that might be a stretch. It is certainly possible to speak of Utumno in a metaphorical sense, as when Gandalf uses the phrase "fires of Udn". But to call it another dimension, outside of the context of a video game, is inaccurate.
