The Extended Edition of the battle would include Holmes somehow managing to grab hold of a ledge while falling, and as Smaug goes down to inspect the remains which he is unable to detect (rather bad pun intended  ) Holmes empties a barrel of molten gold (barrel made of heat-resistant mithril) over Smaug 500ft below. Smaug infers Holmes came from Lake town, and scattering molten gold everywhere, trots off to torch Lake-town. And thus Sherlock got Bard to do the job for him.

Holmes would do that, start something, but then have someone finish it for him after he deems his challenge to be "too boring."
Besides, Holmes already escaped death before.

As far as I know, there's never been a female Sherlock.
No, not in movies or television as far as I am aware. At Eerie Con 2016 (held in Grand Island, NY), author Keith R.A. DeCandido read a short story he wrote that featured a young Shirley Holmes who lives in modern New York City and her newly acquired companion John Watson. The story will appear in a forthcoming Sherlock Holmes anthology.
