The Gladden Fields Isildur (King) was killed (fall) after some two years (short reign) of Kingship. Shortly before his death he gave the Shards of Narsil to Ohtar (changed hands), who removed them to Rivendell. The Ring (Isildur's Bane) was lost in the Anduin (the Gladden Fields).

Didn't mean to be overly blunt, but was in a hurry.... Your guess of Morthond Vale was a good one and hits close to home but doesn't cover all the factors in the riddle.
