How did Gandalf, Sauruman and Grima get onto the roof platform of the Tower of Orthanc?

I have no idea because you can't see a trap door, unless it's disguised in the cosmic-like imagery on the surface. That's my assumption... or that Saruman used the transporter and just beamed in ;)

I have looked - but I cant see the door at all ... nor the lighted numbers for the elevator  ... nor the trapdoor in the floor ... a ladder over the side ... the bungie rope ...

No, no, not the transporter  ! But that it's "hidden" in the design etched on the surface. If the "horns" of Orthanc were wider, I'd suspect a hidden door in one of them, but they seem a bit too narrow for that.
Either that, or Saruman has a very long ladder.  (gets dizzy just thinking about it!)

one that circles round and round and round the Tower and then lands you in the Spa of Orthanc 
maybe theres one of those airline emergency blow-up slides ...
Maybe you just have to fall off - while doing a triple sommersault ... though be sure to remove the spiked wheel ...
Then the spectators at the bottom could have held up score cards for the performance
6.2 ..... 7.5 ...... 8.0 ..... 9.95 .......
I think Sauruman was disqualified on his last dive though ...
